ANNEX 1 - MANDATORY CONDITIONS
The supply of alcohol is prohibited: -
a) At a time when there is no designated premises supervisor, or,
b) At a time when the designated premises supervisor does not hold a personal licence.
c) Every supply of alcohol shall be made or authorised by the holder of a personal licence.
The premises licence holder or club premises certificate holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
CCTV Images will be made available upon request by an officer of the Licensing Authority or the Police.
A CCTV system will be installed, operated and maintained on these premises in accordance with Mid Devon District Councils CCTV Operational requirement document for licensed premises. This document will be completed in consultation with the police and the licensing authority to show the required positioning of all CCTV cameras and CCTV hardware for the premises.
The CCTV agreement within this Operational Requirement document will be signed and dated by the premises licence holder or a person authorised to sign on behalf of the premises licence holder.
The signed CCTV agreement and the completed CCTV operational requirement document or a true copy of the document/s will be maintained at the premises available for inspection by a police officer or an authorised officer of the licensing authority at any reasonable time.
Alterations to the CCTV system that affect the positioning of the CCTV cameras or the quality of the recorded images must be notified to the licensing authority who will undertake a new operational requirement assessment. No alterations to the CCTV system will take place unless agreed by the licensing authority who will document the changes to the CCTV system in an updated operational requirement.
Staff will be trained with regard to their responsibilities in the retail sale of alcohol and regular refresher training will also be undertaken. Training records can be made available for inspection upon reasonable request by the Police or other relevant officers of a responsible authority.
A refusals book will be operated and maintained and will be produced to a relevant officer of the Police or other relevant officers of a responsible authority upon request.
A challenge 25 policy will be operated at the premise, acceptable forms of identification are a passport, photo card driving licence, and PASS accredited identification card.
Spirits will be located behind the counter.
